WH-385-V Certification For Serious Injury Or Illness Of A Veteran For Military Caregiver Leave

Most often submitted alongside the WH-385, the WH-385-V is filled out by both the veteran’s healthcare provider as well as the caregiver in question. This completes the overall form requirement.
